North Carolina Authorization for Cremation& Death Certificate

Cremation Authorization

The legal next of kin, or an individual who has written authorization to make decisions regarding disposition of a loved ones remains, must sign an authorization for cremation form.  This document must be initialed and signed in one of the following ways:

1. In the presence of a NC Licensed Funeral Director during an at need arrangement conference; 

2. In the presence of a Notary Public who will also sign the document and affix their seal; or

3. Initialed and signed in the presence of two witnesses, 18 years of age or older, verifying the original signature.
